2026 Ai-YoH Summer Camp Registration Form

Registration can be completed online through this form, or you may download and print the documents below and complete them by hand. The required forms are found on pages 3 and 4 (Registration Form / Liability Waiver & Parent Consent Form). Completed forms may be submitted in person to Ms. Yin (Principal of Giraffe Learning Center) or emailed as a scanned copy to giraffelearning@yahoo.com.
